Please arrive 10-15 minutes early to fill out registration paperwork, or prepare the paperwork ahead of time by downloading the forms here.

When you come for your appointment, please bring and/or obtain the following:

• Driver’s license or valid government-issued photo ID.
• X-rays, MRI’s EMG’s and any other tests, or at least the reports from these tests. Dr. Yu likes to be able to review the actual studies herself, as well as reading the formal results.
• List of medications (if any).

To cancel or reschedule an appointment

Call the office during business hours and allow at least 1 day’s notice so that we can offer your appointment time to patients on our waiting list. If you miss your appointment, or fail to give 24 hours’ notice of cancellation, a $50 fee will be assessed.

We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies, late patients, or patients with complicated problems require a little more time, and these cause scheduling delays beyond our control. We strive to give every patient the time and attention that they need. We apologize if we keep you waiting.
